PRE-ACCEPTANCE REQUIREMENTS
Prior to the first day of care, the following documents must be returned to Sugar & Spice signed, dated and correctly filled out by the child’s parent, guardian, or responsible party:
_____Physician’s Health Statement (unless child attends school)
_____Current Copy of Shot Record (must be up to date)
_____Copy of driver license of person signing Contract for Service

Full Time =10 hrs. Day Shift ( ends at 6:00 pm )M-F
New born- 17 months
$171.00 per week

Full Time =10 hrs. Evening Shift (ends at 12 midnight )M-F
New born- 17 months $171.00 per week
18 months - 35 Months $120.00 per week
$120.00 per weekPre-School (3, 4, & 5 ) $111.00 per week
$111.00 per weekSchool-Age (6 thru 12yrs) $131.00 per week
Additional Fee: There will an additional charge of $3.00 per hr over 10 hrs per day. Late payment is subject to a $10.00 fee each week.
Parents with rotating and/or changing weekly schedules have to have schedules submitted to management for approval. This will help assure the center is properly staffed for your children.
Before School Care 5:30 am-7:00 am: $40.00 Mon.-Fri.
After School Care 3:30 pm-6:30 pm: $40.00 Mon.-Fri.
Daily Rates : Parents must pre-pay daily fees before care is given.
Newborn-17 months ----$25.00 up to 6 Hrs ----------$35.00 up to 10 Hrs 18 months-older----------$20.00 up to 6 Hrs : ------ : $30.00 up to 10 Hrs
Payment is expected before care is given . If payment is not received by 6:30 p.m. on Monday a $10.00 late fee will be accessed .If your child is enrolled for daily (drop in) attendance and pmt. is not made ,(before care) there will be an additional $5.00 fee that will apply. Your child will be dropped from enrollment until full payment is made.
